Article 2 Application
The main application for compensation under Article 2. The Form is in English and German. Responses to the forms are sometimes found separatley on sheets of carbon paper. Here an example is shown of with the carbon paper laid over the form.
Power of Attorney
This form gave URO offices the right to represent the claimant legally. Several versions of the form were used at different times and offices. Shown here is a form used by the Montreal URO office for Article 2 claims and Hardship Fund Claims.
