Hardship Fund Application (post-1965)

The main application for compensation under the Hardship Fund. The form itself is in English and German but responses are in German. Responses were either typed directly on the form (as shown here) or presented on semi-transparent carbon copy paper in a layout which corresponds to the regions of the form (see below).
 

Hardship Fund Testimony

This survivor testimony was given on carbon copy paper, a method commonly used by URO for Hardship Fund claims.

 

Power of Attorney

This form gave URO offices the right to represent the claimant legally. Several versions of the form were used at different times and offices. Shown here is a form used by the Montreal URO office for Hardship Fund Claims.
